Forms that actually get submitted

Every extra field is a chance to quit. Name, how to reach you, and what you want. That is the default. Budget and timeline can wait for the reply.
The submit button should say where the message goes. “Send to hello@…” beats “Submit”. After send, show a sentence that the mail left, not a blank page. We copy ourselves for two weeks on a new form so we see bounces before you do.
Captchas belong on forms that already attract junk. Putting one on a quiet clinic booking page costs you the grandmother who cannot tell which square is a tractor.
Northline’s quote form is this idea in production: the write-up.
Tell us what you need.
Write a few lines about your project. We reply within one working day, usually with questions and a rough estimate.
